Program for Assertive Community Treatment (PACT)
PACT is a comprehensive service for individuals diagnosed with serious mental health conditions who may benefit from intensive coordinated services. PACT is based on the evidence-based national ACT model, and strives to maintain fidelity to that model.
Services are provided by an integrated, multidisciplinary team providing intensive and comprehensive community-based treatment and supports for individuals living in the community. Services are responsive to the changing needs of individuals served. PACT provides outreach, engagement, rehabilitation, clinical, health-related and recovery-based interventions and supports. The PACT team partners with persons receiving services to facilitate each person’s individual recovery journey.
PACT operates on a closed referral system. Referrals must be made by DMH, MBHP or a OneCare provider (such as Bay Cove). In order to be eligible for PACT services, an individual must be 18 years of age or older; meet DMH service authorization standards; and have the need for intensive coordinated services. The PACT team serves the entire MetroBoston area.
